Blank forms are available for downloading here. They can be completed on your computer. If you saved an electronic copy of your old forms, you can update your old file on your computer. For confidentiality, we recommend you print out the updated form and hand-deliver or mail it to:

What is the policy on illness?
A child with a fever of 100.4 degrees Fahrenheit is sent home, in accordance with the Commonwealth of Massachusetts handbook for Health in Child Care. A child must be fever-free for 24 hours in order to return. We care for children recuperating from diagnosed and treated illnesses and administer prescription and over-the-counter (OTC) pedicatrician-ordered medications with parental consent. When a child is enrolled, families receive a more detailed health policy.

How is behavior management handled?
The LMACCC strives to support children and build self-esteem. The Center maintains consistent safety rules. Specific issues are approached carefully, respectfully and with full parental involvement.

What is the procedure for meals and snacks?
Lunches are brought from home and may be warmed in the classroom. A morning and afternoon snack is offered. Snack may consist of crackers, cheeses, fresh fruits, school-made breads, milk and juices.

Is part-time care available?
The LMACCC provides part-time schedules to families when possible. The schedules vary from classroom to classroom and from year-to-year. Families accepting a part-time position are invoiced at a pro-rated cost; there is an added fee for part-time care.

Is there a sibling discount policy?
The LMACCC maintains a priority waitlist for siblings of children enrolled. There is not a sibling discount.

What are staff qualifications?
LMACCC staff members meet the teaching qualifications of the Massachusetts Department of Early Education and Care. Supervised student teachers from nearby colleges and mentoring volunteers also enrich classroom teaching.
